Repair cross-spec links: rewrite Section guid anchors to relative spec paths when line contains protocol id. Add glossary anchors and repair gt_ links: inject anchors for terms in 1.1 Glossary, map term/abbrev/plural to slug, rewrite gt_guid links to gt_slug.
